Selecting the appropriate check box on the Online Registration Form signifies your acceptance of these terms. You must also bring a signed copy of this waiver to Budo Camp. Anyone who does NOT accept these terms shall NOT participate in this event.
Anyone under the age of 19 yrs old MUST have parental permission to participate. They must bring this form signed by the parent to Budo Camp, or deliver it to Sensei Steiner prior to Budo Camp. Any minor without parental permission AND the below signed waiver will NOT be allowed to participate.
IN CONSIDERATION FOR BEING ALLOWED TO PARTICIPATE IN ANY WAY, INCLUDING TRAVEL TO AND FROM THE MARTIAL ARTS CLASSES, SELF-DEFENSE CLASSES, EVENTS AND ACTIVITIES OF BUDO CAMP, YOU ARE ASKED TO WAIVE YOUR RIGHTS.
I, _____________________,desire to study self-defense and various martial arts, and upon acceptance by Budo Camp, Archdiocesan Retreat & Conference Center, Sensei Shawn M. Steiner, and Steiner Academy of Martial Arts LLC do agree to the following:
1. I acknowledge and understand that I will be voluntarily engaging in strenuous activities that involve contact and the risk of serious injury, permanent disability, or death, and may cause severe social or economic losses due to not only my own actions, inactions or negligence, but also to the action, inaction or negligence of others or conditions of the premises or of any equipment used. Further, I acknowledge that there may be other risks not known to me or not reasonably foreseeable at this time.
2. I agree to forever release Budo Camp, Archdiocesan Retreat & Conference Center, Sensei Shawn M. Steiner, and Steiner Academy of Martial Arts LLC, its instructors, employees, agents, owners, officers, directors, sponsors, other participants, their parents, guardian(s), supervisors and instructors, and the owners, lessors, and lessees of the premises from liability for all claims or losses on account of any injury, including permanent disability and death or damage to property, caused or alleged to be caused in whole or in part by negligence or other acts and for injuries, damages, losses or indemnification resulting or arising from my association with Budo Camp, Archdiocesan Retreat & Conference Center, Sensei Shawn M. Steiner, Steiner Academy of Martial Arts LLC. This release from liability includes, but is not limited to, any injuries sustained as a direct or indirect result of attending or participating in training exercises or any other activities of Budo Camp. I understand and agree that this release and waiver extends to my heirs, successors, assigns, executors, or any other representative or interested party to whom I am obligated at this or any future time.
3. I understand that it is my responsibility to consult with a physician prior to and regarding my participation. I represent and warrant that I am physically fit and have no medical condition, which would prevent my full participation.
4. I hereby agree and covenant not to make a claim against, sue or attach the property of Budo Camp or any of its affiliated organizations or above mentioned parties. I understand and agree that the benefits of this release and waiver of liability extends to Budo Camp, Archdiocesan Retreat & Conference Center, Sensei Shawn M Steiner, Steiner Academy of Martial Arts LLC, and all above mentioned parties, without limitation to the present or future owners, employees, representatives, students, faculty, and all other individuals or business entities of any type who are now, or may in the future be, in any way associated with Budo Camp, Archdiocesan Retreat & Conference Center, Sensei Shawn M. Steiner, Steiner Academy of Martial Arts LLC
5. Parent(s) or legal guardian(s) of minor participants (age 19 and below) additionally agree to instruct the minor participant to the above warnings and conditions and their ramifications, and consent to the minor’s participation.
I HAVE CAREFULLY READ THE ABOVE WAIVER AND RELEASE OF LIABILITY AND FULLY UNDERSTAND THAT I GIVE UP SUBSTANTIAL RIGHTS BY SIGNING IT AND I DO SIGN IT VOLUNTARILY. I AGREE TO PARTICIPATE KNOWING THE RISKS AND CONDITIONS INVOLVED AND DO SO ENTIRELY ON MY OWN FREE WILL.
A parent or guardian must sign for anyone under the age of 19 years.
